The Bassetlaw Social Prescribing Service aims to help patients aged 65 and over who are socially isolated or frail. Referrals are made by your GP to access a range of services and activities provided by voluntary organisations and community groups.

Who can get help from the service?
The service is only available for patients who are registered with a Bassetlaw GP.
How do I get help?
Your GP or Social Care professional will advise you if they feel you could benefit from a discussion with a Voluntary or Community Services (VCS) Advisor.
What does the VCS Advisor do?
Your Advisor will arrange to visit you to discuss voluntary or community services that could help you improve your health and wellbeing. This is in addition to any NHS or social care support you may already be receiving. Your VCS Advisor will also support you to access other services e.g. community transport.
